Online Maintenance

All licensees must maintain their profile and update their information every two years along with a $75.00 Maintenance Fee.  You may update your Maintainenance form and pay this fee online by clicking on the Existing Therapist Logon button at the bottom of this page.

If you choose to update this information by paper and pay by check, download and complete the the Maintenance Form located in the Documents section and send it along with a $75 check to PO Box 2655 Durham, NC 27715.
